2) There is a persistent glitch in the form system that we've encountered over the years. Please read the following thoroughly: if you complete your form and it states that your file upload exceeds the maximum limit, but you've made sure the file is between 3-5 MB, then you must refresh your browser and complete the form again because it won't allow any size, no matter the limit. We apologize for the inconvenience.

3) Please make sure you have your pop-up blocker deactivated when you complete your payment. Use "PayPal Checkout" even if you do not have a PayPal account. It will take you to a pop-up that allows you to choose "Pay with Credit/Debit Card" under the login information.
